Important Before Scheduling
To ensure an appropriate evaluation, veterans must submit the following for preliminary review:
• DD214 (military discharge papers)
• Relevant service treatment records (if available)
• Prior VA decision letters (if applicable)
• Civilian medical records related to the claimed condition
• Imaging or diagnostic reports when available
Incomplete records may delay or prevent scheduling.

Payment Policy for Independent Medical Evaluations
Independent medical evaluations, DBQ-style reports, and nexus medical opinions are private-pay services.
Payment is required prior to scheduling and prior to release of any completed reports. Fees cover the provider’s time for record review, medical evaluation, and report preparation and are not contingent on claim outcomes.
These services are not billed to insurance or the Department of Veterans Affairs.
